When you’re with For Care, you’re in the driver’s seat when it comes to the disability services you receive, and what they look like for you. We’ll always uphold your right to choice and control, and work in collaboration with you. 

Customising a service for you.

When you’re ready to see how For Care can support you, we’ll meet with you to understand exactly what you need and want.

At this meeting, we’ll discuss what your ideal service looks like, information about your funding package, the services we have available to suit you, and how we can tailor a service to meet your specific needs.  

We’ll then prepare some key information to share with you, to help you make your decision. Here’s what you can expect: 

In-home Support, and Social/Community Participation

Supported Independent Living (SIL)

  • Suggested accommodation options: browse some of our current homes to see which ones you like best 
  • Co-tenant profile: learn about your potential new housemates and find common interests

Getting started.

When you’ve decided you’re ready to take your next steps toward independence with For Care’s support – we’ll be ready to help you make your goals a reality! 

If you’re moving in to Supported Independent Living, it’s time to view your potential new home, based on your wishlist – whether that includes a fabulous kitchen for cooking up a storm, a spacious outdoor area, or easy access to local transport and entertainment areas. You can also meet your potential new housemates, and maybe start exploring common interests and activities you could enjoy together.  

We’ll gather some information from you and ask for your funding package information (NDIS Consent to Share form). Then it’s time to get set to start our journey together! 

When you begin with us.

This is an exciting step! When finalising the paperwork we’ll set and plan a commencement date. 

If you haven’t met them already, now will be the time to meet your Service Coordinator, who ensures your service continues to be tailored to your goals.  And if you’re moving into SIL, your Coordinator will be there to meet you on move-in day. 

Ongoing feedback.

As part of our ongoing partnership with you, we want to make sure our services are consistently meeting your expectations and adapting to any changing needs.  

You are encouraged to provide feedback, update your information, and ask questions around your service at any time. We will also ensure we’re offering you the best possible service through:  

  • regular stakeholder meetings: with you, and family members or other specialists who are part of your care team. These are held monthly for SIL, and quarterly for other services. 
  • facilitating quarterly house catch-ups for SIL participants, to check in on how things are going living in a co-tenancy.  

Quarterly check-ins with our Customer Care Manager are also available, to see how your journey with For Care is progressing.
